Telehealth Informed Consent

Last updated: March 2025

What is Telehealth?

Telehealth involves the use of electronic communications to enable health care providers at different locations to share individual patient medical information for the purpose of improving patient care. Technologies used may include videoconferencing, secure messaging, and electronic health records.

Benefits

  • Improved access to medical care for follow-up visits and consultations
  • Convenience of receiving care from your home or office
  • Reduced travel time and associated costs
  • Efficient medication management and dose adjustments

Risks

  • Information transmitted may not be sufficient for appropriate medical decision-making
  • Technology failures may prevent or delay the visit
  • Lack of physical examination may limit the physician's ability to assess certain conditions
  • Security protocols may fail, exposing health information

Your Rights

  • You have the right to withdraw consent at any time
  • You have the right to request an in-person visit at any time
  • Your telehealth visit is subject to the same privacy protections as an in-person visit
  • You may request a copy of your telehealth records

Eligibility

Telehealth services are currently available for Georgia residents only. We are expanding to Tennessee — contact us for updates. New patients should complete an initial in-person visit before transitioning to telehealth follow-ups.
